The Promotion is not sponsored, endorsed or administered by any social media platform.

No purchase or payment of any kind is necessary to enter or win. Making a purchase will not improve your chances of winning. Entry is free by the method described in Section 5.

The Promotion begins on August 3, 2026 at 12:00:00 AM Eastern Time and ends on September 2, 2026 at 11:59:59 PM Eastern Time (the "Promotion Period"). All times are Eastern Time. The Sponsor's clock is the official timekeeper for the Promotion.

Entries submitted before or after the Promotion Period will not be eligible.

Open only to legal residents of the fifty (50) United States and the District of Columbia who are eighteen (18) years of age or older at the time of entry.

Employees, officers and directors of the Sponsor and of CoreAge Rx, and their respective affiliates, subsidiaries, advertising and promotion agencies, and the immediate family members and household members of each, are not eligible.

Void in Puerto Rico, all other US territories and possessions, and wherever otherwise prohibited or restricted by law.

During the Promotion Period, submit your email address through any giveaway entry form on https://www.healthconnectalliance.com and agree to these Official Rules. Entry is free.

Agreeing to receive marketing email is optional, is not required to enter, and does not affect your chances of winning. Withdrawing marketing consent later will not cancel a valid entry.

Limit one (1) entry per person and per email address for the entire Promotion Period. Duplicate entries will not be counted more than once.

Entries generated by script, macro, bot or any other automated means, and entries submitted through any method not authorised in these Official Rules, are void and will be disqualified. Attempting to enter using multiple email addresses or identities will void all of that entrant's entries.

All entries become the property of the Sponsor and will not be acknowledged or returned. Proof of submission is not proof of receipt.

One (1) prize will be awarded: a $1,788 CoreAge Rx Gift Card.

Approximate Retail Value ("ARV"): $1,788 USD. The ARV is the face value of the gift card. Total ARV of all prizes: $1,788 USD.

Only one (1) prize will be awarded per person and per household. The prize is not transferable and no substitution or cash equivalent is offered, except that the Sponsor reserves the right to substitute a prize of equal or greater value if the advertised prize becomes unavailable.

The gift card may be applied toward eligible products or services available for purchase through CoreAge Rx, subject to the Gift Card Terms and to applicable product restrictions. The Gift Card Terms are incorporated into these Official Rules by reference.

Not every product or service is available to every person. Availability may be limited by geography, by clinical suitability and by applicable law.

Winning does not guarantee eligibility for, approval of, or access to any prescription medication.

Where the winner chooses to apply the gift card toward a prescription product:

• The winner must complete the applicable intake process.
• The winner must be evaluated by a licensed healthcare provider.
• Eligibility and all treatment decisions are made solely by the licensed healthcare provider.
• The gift card cannot override, influence or substitute for a provider's medical judgement.
• No particular medication is promised, and no medication is guaranteed to be available.
• Compounded medications are prepared by licensed compounding pharmacies and are not FDA-approved products.

Nothing in this Promotion is medical advice, and no health or weight-loss outcome is promised or implied.

One (1) potential winner will be selected in a random drawing from among all eligible entries received during the Promotion Period. The drawing will be conducted on or about September 9, 2026 by the Sponsor, whose decisions are final and binding in all matters relating to the Promotion.

Odds of winning depend on the total number of eligible entries received during the Promotion Period.

The potential winner will be notified by email at the address submitted with the entry, within three (3) business days of the drawing.

The potential winner must respond to the notification within five (5) days of the date it is sent. Failure to respond within that period, an undeliverable notification, or a failure to meet any eligibility or verification requirement will result in disqualification and forfeiture of the prize.

If a potential winner is disqualified or forfeits, an alternate winner may be selected at random from the remaining eligible entries, subject to the same requirements. Up to two (2) alternate drawings may be held, after which the prize may go unawarded.

All potential winners are subject to verification of identity, age, residence and eligibility before a prize is awarded. The Sponsor may require the potential winner to sign and return an affidavit of eligibility, a liability release and, where lawful, a publicity release within a stated period.

The Sponsor reserves the right to disqualify any entrant it reasonably believes has tampered with the entry process, violated these Official Rules, or acted in an unsportsmanlike or disruptive manner.

All federal, state and local taxes on the prize are the sole responsibility of the winner. The winner may be required to provide a completed IRS Form W-9 and a valid taxpayer identification number, and an IRS Form 1099 may be issued where required by law. The winner is advised to consult a tax professional.

Except where prohibited by law, acceptance of the prize constitutes permission for the Sponsor to use the winner's first name, last initial and state of residence for promotional purposes without further compensation. Residents of jurisdictions where such consent may not be conditioned on prize acceptance may decline without forfeiting the prize.

By entering, entrants release and hold harmless the Sponsor, CoreAge Rx, and their respective affiliates, officers, directors, employees and agents from any liability for any injury, loss or damage of any kind arising from participation in the Promotion or acceptance, use or misuse of the prize.

The Sponsor is not responsible for late, lost, misdirected, incomplete, garbled or undeliverable entries or notifications; for technical failures of any kind; or for any error, omission, interruption, defect or delay in transmission or communication.

Nothing in these Official Rules limits liability that cannot be limited under applicable law.

Information submitted with an entry is collected by the Sponsor and handled in accordance with the Privacy Policy, which is incorporated by reference.

Entry data is used to administer the Promotion. It is used to send marketing email only where the entrant has separately opted in, and that consent may be withdrawn at any time without affecting the entry.

These Official Rules are governed by the laws of the United States and of the state in which the Sponsor is domiciled, without regard to conflict-of-law provisions. Entrants agree that any dispute arising out of the Promotion shall be resolved individually, without resort to any form of class action.

Any dispute that cannot be resolved between the parties shall be submitted to the exclusive jurisdiction of the state or federal courts located in the Sponsor's domicile.

If the Promotion cannot be conducted as planned — because of infection by computer virus, bug, tampering, unauthorised intervention, fraud, technical failure, or any other cause beyond the Sponsor's reasonable control that corrupts or affects the administration, security, fairness, integrity or proper conduct of the Promotion — the Sponsor reserves the right, at its sole discretion, to modify, suspend or cancel the Promotion, and to select the winner from among the eligible entries received before the action was taken.

The Sponsor also reserves the right to modify, suspend or cancel the Promotion where required to comply with applicable law.
