The Cheapest Way to Get Brand-Name GLP-1s in 2026

The cheapest way to get FDA-approved GLP-1 medication in 2026 is usually not a telehealth program. It is a prescriber plus a manufacturer-direct pharmacy — and the difference can be over a thousand dollars a month compared with buying the same drug through a platform. Here is how the arithmetic works.

LillyDirect self-pay pricing for Zepbound

DoseMonthly self-pay price
2.5 mg (starting)$299
5 mg$399
7.5 mg – 15 mg$449 flat at every dose

The $449 rate on 7.5 mg and above requires purchasing each refill within 45 days of your previous delivery. As of spring 2026, KwikPen self-pay pricing also works at major retail pharmacies through the savings card, not only through LillyDirect.

The two-part configuration

Manufacturer-direct programs do not prescribe. You need a clinician, and that is the second half of the setup. The cheapest legitimate structure looks like this:

  • Prescriber: a low-cost telehealth membership. PlushCare runs $19.99 a month or $149 a year with the first month free, and bills insurance for visits in many plans.
  • Medication: LillyDirect at $299 to $449 depending on dose, or Novo's direct channel for Wegovy.
  • Everything else: GoodRx for your adjacent prescriptions, which is free and occasionally beats an insurance copay.

At maintenance dose that totals roughly $470 a month for FDA-approved tirzepatide with a board-certified physician relationship. Compare that against telehealth platforms charging $1,498 to $1,839 for branded GLP-1s — the same drug, roughly triple the price.

When a bundled program is still the better choice

The two-part setup is cheaper. It is not always better, and there are three situations where a bundled program wins clearly.

You have insurance worth fighting for. If your plan covers GLP-1s, prior authorization is the barrier, and a platform with a dedicated insurance team is worth far more than a low membership fee. A $25 copay beats $449 by a wide margin.

You need clinical support during titration. The two-part setup gives you a prescriber, not a titration partner. If you have GI history or have quit a GLP-1 before because of side effects, a program with real clinical contact is the variable that changes your outcome.

You want semaglutide, not tirzepatide. LillyDirect is tirzepatide only. Novo operates a direct channel for Wegovy, but check current pricing — the tiers differ.

Compounded is still cheaper — how much cheaper

Compounded semaglutide starts around $99 a month at the cheapest verified providers, and compounded tirzepatide around $167. Against $449 for brand-name Zepbound, the gap is $280 to $350 a month, or roughly $3,400 to $4,200 a year.

That is a real difference and for many people it is decisive. What you give up is FDA approval, batch testing, and bioequivalence data. Both choices are defensible; the point is to make it knowingly rather than by default.

Watch the 45-day refill window

The flat $449 rate on higher Zepbound doses only holds while you purchase each refill within 45 days of your previous delivery. A month is about 30 days, so you have two weeks of slack — enough for ordinary life, not enough for a long trip or a deliberate pause. Set a recurring reminder at day 30 and treat it as a standing obligation of the program.

Bottom line

If you are paying cash and want FDA-approved medication, pair a cheap prescriber with a manufacturer-direct pharmacy. If you have insurance, use a platform that will fight your prior authorization. If cost is the binding constraint and you accept the trade-offs, compounded remains the cheapest route.
